1. By completing the Grotto Rewards enrollment form, you agree to the terms and conditions of the program. Terms and conditions may change at any time
2. You must be at least 18 years of age to enroll.
3. Gift cards that are lost, stolen or expired cannot be replaced.
4. Only the member paying with a registered credit or debit card may earn points.
5. Points are earned for purchases of food, beverage, retail sales, gift cards, and on tax & gratuities.
6. One reward point is earned for every $1.00 spent using registered payment cards - including tax and gratuity.
7. Points have no cash value.
8. Points are not transferable.
9. Gift cards redeemed do not earn points.
10. Cash transactions do not earn points.
11. Purchases prior to the date of enrollment do not earn points.
12. You may update your personal profile, register additional cards, and redeem points by logging into www.GrottoOakBrook.com.
13. Points may not be earned on discounted items.
14. Grotto reserves the right to change or cancel this program at any time.
15. Reward points will expire if you do not have at least one visit using any of your registered cards within a 12 month period from your previous visit.
